Completion of any of these forms does not constitute any reservation/agreement/ until which time all fees are paid and you receive a receipt from our office. Do not make any plans based on the completion of these forms until you have confirmed payment with our staff at the district office at 4855 Hamilton St., Sacramento, CA 95841.
